DXA Scan patients – if you are new to our practice or have not been seen in the last 3 years, you must complete the four new patient forms listed above, in addition to, the DXA Risk Assessment Questionnaire. Current patients of Carroll Arthritis, P.A. need only complete the Risk Assessment form.
